The achievement window tracks long-running goals across your whole character. It is the same machinery the battle pass runs on, which is why the two feel alike.

Trophies, rewards and titles
Each achievement carries a trophy tier - bronze, silver, gold, platinum, or diamond at the top - and every one of the 1,546 enabled achievements now carries a reward you claim from the window once the objective completes. Some also grant a nickname, a title you can display.
Achievements often run in chains, where finishing one opens the next, and the diamond entry is the end of its chain.
An entry you have not finished yet shows the medal it awards, dimmed - not a question mark - so you can see what a rung is worth before you go after it.

Two passes landed. First, 211 entries that granted nothing at all were given a reward. Then every reward on the board was rewritten to match what its tab is actually about:

Three things were removed everywhere. Time Stones I-X had no use effect and no contents, so they could never do anything; the 87 dungeon tickets were the same, since every entrance condition only ever reads a coin, a level or a quest - which is why the dungeon rungs pay a coin now. The random achievement boxes are gone too.
Only potions that restore on the spot are handed out. The "Potion of Vitality / Mana" family is a 27-second regeneration rather than a heal, so it is no longer used as an achievement reward.
The Codex pays for the first time. Every Codex entry now says what to do to record it instead of only its level, and each one pays a potion for that level. Six dungeons the Chapter tabs never listed - Goblin Deepworks, Court of the Faunus King, Fugitive Palace, Marionette Hall, Caucasus Rock Pit and Temple of Agony - now have an achievement of their own, one clear each on any difficulty. Forty-three Codex entries that pointed at unnamed items rendered as "-" and could never be completed; those are switched off.
Anything you had already finished is waiting for you. The new rewards are claimable retroactively from the Achievements window.
The quirk worth knowing
Progress is kept on the server while you play and written to the database when you log out. In game your counters are always live. Anything outside the game that reads your progress - a website page, an external tool - is only as fresh as your last logout.
So if a tracker looks stuck while the in-game window is clearly counting, nothing is broken. Log out and it catches up.
